A part time (30 hours), permanent Care Manager 1 vacancy has arisen within the North Locality Adult Social Care Team of the Community Services Business Unit based in the Berwick District Office

As a Care Manager 1, you will have access to excellent and regular formal and informal supervision, annual appraisals, support and guidance to promote your personal and professional development.

Main duties of the job

To manage and plan such work in an effective way as to allow the required monitoring and review of services delivered.

To have continued responsibility for the management of own caseload, seeking guidance from Line Manager as appropriate.

To undertake Carers’ Assessments as part of the care management process.

To carry out Risk Assessment as part of the care management process.

To participate in the Duty System, taking and processing information and acting on them under supervision as required.

To participate in discharge planning and care planning meetings.

To ensure all written and electronic records are maintained to Trust standards.

To provide written reports, outcomes of assessments and recommendations.

To advocate on behalf of service users.

In line with the new transformation agenda, to carry out assessments and offer Personal Budgets, seeking guidance from Line Manager as appropriate.

Detailed job description and main responsibilities

• To provide care management intervention with adults across the specialisms of Mental Health, Learning Disability and Physical Disability.

• To work within a Team where a diverse range and complexity of social care and health issues are presented.

• To seek guidance when required and formal supervision from line manager and senior practitioner, in line with departmental policy.

Qualifications
Knowledge acquired through in house training, short course and experience to NVQ 3 level or equivalent plus knowledge of specific patient conditions, forms of therapy, acquired through training, experience
Working with adults in a social care setting
