An opportunity has arisen for enthusiastic individuals to join a dynamic community team, consisting of care and therapy staff. The Short Term Support Service offer assessment, rehabilitation and re-ablement to support clients who may have experienced a short-term illness, or event e.g. fall that has impacted on their health, or a deterioration in an existing condition. The service plays a key role in facilitating safe timely discharge from hospital, and also in supporting people to stay at home to prevent hospital admission.

The successful candidate will deliver care and support to a diverse range of clients within their own homes, with a view to optimising their independence. All care will be delivered to a very high standard and in accordance with the core values of the organisation.

The role requires good communication and literacy skills, and the candidate must be flexible, compassionate and willing to learn new skills. The successful applicants will have a minimum qualification of NVQ level 2, or be committed to achieve The Care Certificate Award, and also possess the ability to undertake further identified specialist training to meet the requirements of the post.

It is also essential that the successful candidate can fully meet the transport requirements of the post, for which expenses will be paid in accordance with the organisations Single Status agreement rates. The team operates across a wide mainly rural area, 365 days a year on a shift basis between the hours of 07:00 and 22:00hrs.

Risk Assessments are in place with regard to Covid 19 infection control, ensuring compliance with government and corporate guidance to protect staff and clients.

Contracted hours will be 30 hours per week across a seven day rota. The salary is £16290 - 17633
