This is an opportunity to be part of Newcastle’s Cardiopulmonary Transplant Service, which specializes in heart and lung transplant for people with heart failure or life-threatening lung disease. Patients can be from anywhere in the UK. As a specialist social worker, you would be required to:
• Complete social circumstances assessments to inform MDT decisions on transplantation
• Evaluate risk arising from psycho-social factors
• Participate in MDT decision-making
• Give advice and support to people who are to receive a transplant, who are awaiting a suitable donor, or those who have received donor organs and are in recovery, on matters that affect their social circumstances, such as benefits or housing
• Engage with other local authorities and CCGs to support people to access necessary services and support in their locality
The successful candidate would be one of two social workers in a predominantly NHS multi-disciplinary team but would also be based in a wider hospital social work team which is part of Newcastle City Council. Line management and supervision is provided by an experienced social work manager.
