An exciting opportunity has arisen for someone to join our team supporting the implementation of the national hospital discharge and community support policy. The role requires strategic and operational abilities to support the discharge to assess and home first principles across the city of Newcastle.

The successful candidate will be a suitably qualified professional, who is able to make decisions to support the best possible outcomes for patients as they're leave hospital to continue their recover at home, wherever possible. The role will lead on the strategy around the flow of patients between hospital and community settings as a key aspect within the wider health and social care integration agenda.

The role is initially fixed term for 2 years therefore, secondment opportunities will be considered, please ensure that you have permission for a secondment from your current employer prior to applying. The employing organisation for this role is Newcastle City Council.
