An exciting opportunity has arisen for 2 social workers to join the Care and Support service in Adult social work services to be based in Eddie Fergusson House in Blyth

The successful candidates will work as part of an experienced and highly skilled tea, within the Care and support service They will work with a range of people who use our services who have complex and diverse physical health needs, providing direct services or arranging for therapeutic services. This includes undertaking statutory, care management and safeguarding adult’s duties

One of the posts is for a McMillan social worker who will work with clients that have a palliative diagnosis, working with a multi disciplinary team to offer a holistic and humanistic approach in working with people and their families at this critical stage in their lives.

The successful candidates will each carry a complex mixed caseload of service users in a variety of circumstances aiming to achieve good outcomes for service users and advocate on their behalf. Duties include assessing service users and their Carers; to devise person centred plans which address their identified needs. Assess and managing risk utilising an evidence based approach. Including the monitoring and reviews of care plans.

The successful candidates will be skilled in using a range of social work tools, practice and research to underpin their interventions. Working alongisde a range of professionals from different specialisms and settings working directly with service users and their families ensuring the welfare of the service user promoted.

The candidates will be skilled in establishing and maintaining effective working relationships with individual service users, groups, families and multi-agency partners improvement agenda. The candidates will participate in the office duty system as required.

Qualifications, knowledge and skills required: Degree in Social Work, Dip SW, CQSW, CSS. Completed Assessed Supported Year Employment Current Social Work England Registration. Up-to-date understanding of the key issues and relevant theoretical background facing professional Adult social workers, particularly related to Safeguarding Adults and Mental Capacity Post Qualifying Awards in Social Work (and related professional qualification training) ie Practice Educators and or Safeguarding. Associated training relevant to the specific post Up-to-date knowledge of relevant research and legislation Hospital Discharge Policy Experience Substantial experience of working with Adults. Experience of Safeguarding Adults. Experience of team working. Experience of positive decision making. Experience of working in a range of social work settings.

The successful candidates will have the ability to form positive relationships with service users and colleagues. Demonstrable risk assessment and critical thinking skills. Be able to communicate effectively both verbally and in writing with service users and other professionals.

They will be Able to work under pressure, meet deadlines and have strategies to cope with own stressors. and be competent with ICT systems and programmes.

There is an expectation that the candidates will have a flexible approach to the hours of working to accommodate changes in working patterns at short notice. To implement the principles of agile working, in line with Northumberland County Council’s New Ways of Working.

They will have a willingness to attempt new challenges and approaches and a positive attitude to supporting equality and diversity. To be committed to meeting the needs of service users through collaborating with colleagues and other professional services. The ability to listen and understand the needs of service users and their Carers.

There is an expectation of some lone working in the posts and a full driving licence with full business insurance cover is necessary to be able to satisfy the mobility requirements of the post.
