An opportunity has arisen for a qualified Social Worker to join the Community Health and Social Care Direct Team. This is a friendly and supportive team committed to person-centred practice which helps promote independence, wellbeing and choice.

The Community Health and Social Care Direct Team provide the first point of contact for Adult Social Care. Your role will include responding to crisis/emergency situations, completing adult assessments and carers assessments. Part of the role involves providing information, advice and support to prevent the need for long term support arising and promote independence. The role requires you to be flexible, adapt quickly and have the ability to make sound judgements, often in an emergency.

The applicant should have a relevant social work qualification, such as Degree in Social Work, DipSW, CQSW, CSS or equivalent, and must have current SWE registration. Ideally the applicant should have post qualifying experience in an adult social care setting.

Professional support, supervision and appraisal will be provided by the Team Manager of the Community Health and Social Care Direct Team.
