Fixed Term contract for 3 years due to Funding

An exciting opportunity has arisen for a Health and Well-being Coach to be part of our leaving care team and work closely with our relevant partners. Following a successful focussed Ofsted visit in March 2022 to the integrated team, the inspection highlighted that workers are dedicated and committed and are clearly proud of our young people and that most young people supported by the service have long lasting, trusting relationships with PA’s and support is tailored to young people’s individual needs and wishes.

This is an exciting time to work for South Tyneside Council where there is a real commitment and passion in delivering outstanding services to care leavers. Through a culture of innovation and DFE funding we have been successful in securing the resources to further bolster and develop our offer to young people via an enhanced staying close model and framework.

This role would support the success of the staying close model and add unique diversity and skills to the team from a health perspective and intervention. You will provide responsive emotional and psychosocial support to care experienced young people and provide life skills support, run well-being sessions, and support our peer mentors.

As a Health and Wellbeing Coach you will work with care experienced people to support them to develop their knowledge, skills, and confidence to become active participants in looking after their own health.

You will guide and support people to reflect on and change their health-related behaviours to help them reach their self-identified health and wellbeing goals. You will be skilled in coaching, communication, and behavioural change skills and able to work alongside people (individually or in groups) at their starting point to encourage and support them to become more engaged in managing their own health.

We want to work closely with our emerging Integrated Care System in developing trauma informed well-being practice and this role will be crucial to supporting this aim.

As we are committed to enhancing the service and offer to young people, the role will require a flexible approach to providing support to young people outside of office hours as required.
