About the Role: To further strengthen our Learning Disability and Mental Health Teams we are looking to recruit 2 additional Senior Social Work Practitioners to work with people on a targeted approach in order to deliver better outcomes through improved accommodation and support options. Working as part of a virtual project team, the roles will work with housing, commissioning, health, service improvement team and business intelligence colleagues to deliver better outcomes for people.

The roles will include providing intensive early intervention, reviewing high-cost packages of care and support, supporting improvements in service delivery and quality assurance as well as providing additional capacity for social work leaders during periods of change.

About You: Driven by research, innovation and the desire to develop best practice, you must demonstrate an applied knowledge of Adult Social Care legislation and the NHS Continuing Health Care Framework and be willing to undertake further bespoke learning and development.

Registered with Social Work England and having significant post qualification experience, you will want to continue practicing in a setting that believes that people are at their healthiest and happiest when they are supported to remain independent, close to the people they love and who love them, in the neighbourhoods and communities they call home.

About South Tyneside: South Tyneside has a rich heritage and our history has played a major role in regional, national and even world history. It was a military strong hold for the Romans and an important centre for the early Christian church, where the Venerable Bede wrote his “Ecclesiastical History.” An area that was rich in coal with a key position on the River Tyne, it has a proud industrial and maritime past and was at the forefront of invention and innovation in these areas. Our Director of Public Health uses hypothetical children Jack and Emily to demonstrate the differences the two children born on the same day, but from different parts of the borough, and what impact that had on their health and wellbeing as they developed as children into adulthood. Jack was born in the ward of Simonside and Rekendyke and Emily in Cleadon and East Boldon Ward. Their journeys through life are quite shockingly different, culminating in jack expecting to live 75 years and Emily 87 years; a whole 12 years longer.
