We have an exciting opportunity for a Part Time 30 hours per week Social Work Support Assistant on a permanent basis within the Home Safe Hospital Discharge Team based at Wansbeck General Hospital.

The post will work alongside adults in hospital who have a physical disability or illness, mental health and learning disability. This is an exciting opportunity to work within a fast-paced team with enthusiastic and experienced Social Work Support Assistant and Social Workers. The team has close working relationships with Community Care and Support Teams and both primary and secondary Health Services.

The post holder will be required to undertake Urgent Needs Assessments and to devise care plans, which address needs in line with the Care Act 2014 eligibility criteria, taking into account individual service user choice, physical, emotional, psychological and environmental circumstances.

Whilst this post is predominantly based at Wansbeck General Hospital the applicant may be required to support other sites within the County.

The hours are currently Monday – Friday 8:30am to 5pm, however there may be future service development to include weekend working to provide a 7 day service.

The main responsibilities include:
- Carrying out assessments and working with service users to design solutions to meet their desired outcomes, seeking guidance from your Line Manager and colleagues where appropriate.
- Participating in discharge planning (from hospital), Ward Rounds and care planning meetings.
- Managing and planning such work in an effective way to allow the required monitoring and review of the outcomes achieved for an individual.
- To undertake non-complex Mental Capacity Assessments
- Carrying out Risk Assessments as part of the care management process.
- Take part in shadowing of students, e.g. Social Workers, Occupational Therapists and newly appointed staff from other disciplines.
- Ensuring all written and electronic records are maintained to Organisational standards.
- Providing written reports, outcomes of assessments and recommendations.
- Advocating on behalf of service users.

Essential requirements:
- NVQ 3 in social care or equivalent
- Experience of working with adults in a social care setting
- Some knowledge of legislation relevant to mental health and adult social care
- Some knowledge of, or experience of, working with people experiencing mental illness, learning disability and/or physical conditions in adults and older people
- Due to the nature of this role, it is an essential requirement of the role that the post holder has a valid driving license and has access to a car they can use for work purposes
