We are looking for an experienced leader to work in partnership between social care practitioners and service providers. Supporting accurate care planning and recording of service and financial information.
About the Role:
The Independent Supported Living (ISL) Facilitator will support operational social care teams with support planning to ensure the accurate procurement of services, and accurate recording of information on the social care system (LAS).
The post holder will have responsibility for mapping, coordinating, and procuring 1-1 and shared support within all ISL accommodation, liaising with people and social work staff to ensure the best use of resources.
You will work with providers to confirm care plans and start dates and liaise with Sub Contractors where necessary to ensure correct information is communicated.

About You:
You will have experience of working within an independent living service (ISL) or social care environment or similar people focussed organisation. Experience of building positive relations with a wide range of professionals, members of the public and people who use services and their families.
You will have the ability and experience to develop and maintain robust monitoring and data collection systems.
You will be passionate in the delivery of high-quality services, enabling people to live better lives. You will be passionate, innovate, professional and a real team player.
