Do you have experience of facilitating and coordinating health and social care systems and processes?

Are you innovative and have a passion to make a difference to the lives of people in need of social care?

We are looking for an experienced leader to work in partnership with our domiciliary care providers to help drive continuous improvement, promote best value, and deliver high quality support for the people of South Tyneside.

Working with our Trusted Assessors and care providers you will be using evidence-based decision making to identify the most proportionate package of support to safely meet the ongoing needs of the person to maximise independence and support them to meet their outcomes.

About the Role:

As the Trusted Assessor Facilitator, you will support the operational social care teams in a hybrid capacity by adopting the ‘Let’s Talk’ approach to inform support planning activities to ensure the accurate brokerage of services based on the 3 conversations model is effectively used. The Let’s Talk approach ensures the person is at the centre of their decision making, as experts in their own lives. The Trusted Assessor Facilitator will also ensure accurate recording of information on the social care system (LAS).

About You:

You will have experience of working within a social care, health, housing, or other relevant environment with people with support needs. This may include people with a learning disability, autism, older people, and people with physical and mental disabilities.

You will have the ability to analyse data to inform evidence-based decision making and to understand and correctly assess levels of need in order to formulate support plans, with clear measurable outcomes. You will have experience of data inputting or coordination of services.

You will be passionate in the delivery of high-quality services, enabling people to live better lives. You will be passionate, innovate, professional and a real team player.
