The Service
Northumberland Communities Together provides help and support to community organisations and individuals across the County. The team encourage an asset-based approach, assisting local community organisations to build upon what’s strong and deliver activities and local services for themselves.

By working alongside communities and in collaboration with local partners and community organisations, the team aim to help create positive change which is driven and led by the community to meet local needs.

The team also provide support and advice to meet the welfare needs of individuals.

Alongside, this support, is an innovative programme of events and actives, delivered from our community hubs and local venues to provide social support, improve health and wellbeing and encourage opportunities for volunteering. 

The Role
Northumberland Communities Together is seeking to appoint two Support Planners to join a highly skilled team who are focussed on delivering the highest possible standards of service to local residents.
This post is permanent.

As a Support Planner you will assist residents in Northumberland to make informed choices as to how they wish to arrange their care and support to meet the outcomes identified through a need's assessment. Support Planners will also offer advice and assistance to people who are full fee payers but who have self-assessed that they need help to arrange their support, giving options, choice, and control.
In addition to responding to the needs of individual residents, Support Planners will collate a detailed understanding of local support services available within the community and collaborate with community organisations to identify gaps in provision.

The team have an enviable reputation in being responsive team, flexible and delivering creative solutions to the needs of residents in partnership with social care, primary and secondary care.
