Applications are invited from suitably qualified and experienced persons who have worked with adults for the post of reablement officer within a community and residential setting.
 
Are you interested in working as part of a multi-disciplinary team? If so then this might be the position for you.
 
You must be self-motivated and have a flexible approach to working arrangements and have an understanding of working in the community and in an individual's own home.  
 
You must have a genuine interest in promoting independence and supporting an individual to regain confidence, so that they can return home after a period of rehabilitation within a residential setting.
 
You must have a full clean driving licence and access to a vehicle.
 
The main areas of responsibilities of the post include:

Under the guidance of qualified therapist's to provide individual activities as part of the assessment and treatment programmes for the individual.
 
To take responsibility for the administration of medication and the necessary recording that takes place.
 
To undertake risk assessments when requested to do so.
 
Working without direct supervision and responsible for individual progress and reporting back to the lead person on individual's performance, behaviours, strengths, needs, concerns and suggested progressions.
 
To liaise with other professionals, e.g. GP's, consultants, district nurses, social workers, care agencies and other allied health professionals when the need arises.
