We have created a new Social Work post within the Adult Access Team, based at South Tees Health and Social Care Single Point of Access (SPA) to support the teams operational capacity.

The Access Team is the main point of contact for new requests for assessment, advice and information from the citizens of Middlesbrough.. The role is varied and will enable the successful candidate to develop a focus on screening, prevention, sign posting, asset based approaches and reablement to support individuals to achieve their goals and desired outcomes.

Undertaking assessments of individuals and their carers you will focus on meeting identified needs through a range of options including direct payments and commissioned services where unmet need remains.

The role is both interesting and varied as it covers all areas of adult practice. The teams strength comes from the knowledge and experience individual team members bring to the team, whilst enabling them to develop skills in new areas of practice.

The team is co-located at the SPA which is a base for Middlesbrough Council, Redcar and Cleveland Council and health colleagues. You will be joining us at an exciting time as we develop services to maximise the benefits of integrated working between partner agencies.

Applications are invited from qualified Social Workers, registered with HPCP who are able to demonstrate:

Experience in adult social care
Excellent written and verbal communication skills
Experience in managing complex caseload
Adult safeguarding experience: information gathering / investigation
Experience in positive risk management
Enthusiasm and motivation to work with adults in need
