Applications are invited from qualified Social Workers for a number of vacancies, which have recently become available within Sunderland in the Community Locality Based teams and the Hospital SW team
Principal duties will be to complete assessments of need working to the National Eligibility Criteria as introduced by the Care Act and the arrangement of Care Support for vulnerable adults aged 18 and over, working closely with statutory partners, independent agencies and third sector partners.
