Grace House provides short break accommodation to children and young people between 5 and 17 years of age who have a range of disabilities including learning and physical disabilities, Autism and behaviours which might challenge and require differing levels of support to meet a range of social and sometimes complex health needs.
Our short break service requires dynamic motivated senior support workers with the ability to work in challenging environments. Senior Support Workers will need to be motivated to engage children and young people in line with positive behavioural support to enable a positive outcome and be willing to undertake training to support children and young people with a range of health interventions.
You will need to work effectively as part of a team, within a multi-agency framework, providing care and support to enable young people to achieve their maximum level of independence through appropriate physical, emotional and psychological support including: intimate personal care, support with a range of health needs, practical tasks, appropriate communication and community involvement.
You will be required to assess, coordinate and provide personal and practical interventions to care and support young people in line with individual care plans, including liaising with families and health and social care professionals.

• A starting salary of £17,572 rising to £19,021 (pro rata, based on 33.25 hours)
