We are looking for an experienced, committed and enthusiastic individual who can demonstrate and undertake the initial assessment processes and preparation of Reablement Support Plans and associated co-ordination of social care aspects of hospital discharges into the Council’s reabling services in accordance with Adult Social Care pathways for hospital referrals.

You will work under the direction of the ART/MDS Team Manager in a small team of Co-ordinators and be involved in triage referrals into Adult Social Care from all hospital wards where a persons’ stay in hospital has been 72 hours or more liaising with Health colleagues, the person and where appropriate family /carers to determine the potential for reablement support or the need for a long term care package.

You will be qualified to Level 4 Diploma in Health and Social Care (or will be expected to achieve this standard on appointment). You should have a general understanding of the legislation and good practice that underpins adult care, in particular the Care Act (2014) and issues associated with ordinary residency and prevention.

You should be confident in using ICT (Care Director) and Word, Excel and Outlook (Microsoft). You should have experience of working directly with adults and professionals in a health or social care environment and knowledge and experience of assessment and support planning for adults to maximise their independence and reablement support requirements.
