As a Social worker, your role will be to ensure a high-quality response, the most effective use of resources and to build on existing, positive relationships with our partner agencies.

Within the Older People and Physical Disabilities Service, which provides assessment services to both Hospital and Community based users, you will have the day to day responsibility of providing assessments of need and care support planning with a wide and diverse range of people and their carers.

You will have extensive knowledge of the Care Act and a demonstrated understanding of the needs of customers and carers.

There is commitment to personal development and the opportunity to contribute towards the strategic direction of the department through involvement in working groups.

This post is exempt from the rehabilitation of Offenders Act 1974 and therefore will be subject to a DBS check from the Disclosure and Barring Service.
