Post title: Edge of Care Community Hub Worker – various part time, full time, permanent and temporary posts available

Job Location: The post is based at Nettlesworth, however you may be required to work at any DCC council office.

This role includes supporting families with multiple and complex needs, using a variety of approaches to assist in rebuilding relationships.

The team offer a range of restorative interventions to encourage family decision making and responsibility for their children and young people by negotiating and discussing support within their family network to ensure the safeguarding of children and young people.

This is a seven day service (not including Bank Holidays) and staff undertake some evening and weekend work to provide this.

Staff receive a wide range of training to develop their role in supporting families.

The service is countywide and based at Nettlesworth alongside the Emergency Duty Team.

Edge of Care Community Hub Workers offer flexible support to existing placements in the community in order to prevent the admission of children into local authority care. They provide rapid response and intensive interventions to families where children are on the edge and edging towards care.

Staff offer support and care to young people looked after within the out of hours residential establishment in accordance with the policies and procedures of Durham County Council. This will include providing services to young people and families when required which includes evenings and weekends.

Mediation and a range of restorative interventions is offered to families within their family network to ensure the safeguarding of children.

Required qualifications: Level 3 Health & Social Care or equivalent.

The successful applicant for this post will be required to apply for a Disclosure & Barring Service Enhanced Disclosure.
