The 18-25 Disability Team have an exciting opportunity for a Social Worker(s) to join our dynamic and dedicated team. The role will involve working closely with our partners within Childrens Services, Health, Education and Specialist Services to ensure a smooth transition for the young person and their families.
The post holder will play a key role in ensuring young people who may be eligible for Adult Social Care under the Care Act 2014 have an effective transition from Children’s Services. The role will include joint working with Children's Services and SEN, undertaking social care assessments and support planning; monitor and review packages of support There will be an opportunity to carry out developmental work, working within the legislative frameworks (Children and Adults) and agreed Local Authority procedures.
You should have recent relevant experience of working in a Social Care Management setting dealing with enquiries from the public or agencies. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, ideally including knowledge of current developments and service pathways.
You will need good administrative skills and a proven experience of developing, maintaining and inputting information on electronic based systems. You will be able to demonstrate sound analytical skills and effective decision-making within appropriate timescales; demonstrate a person-centered approach and be able to act in a tactful and sensitive manner with people who access services.
