An exciting opportunity has arisen for a Social Worker to work within the Early Intervention and Prevention Social Work Team based at Tithebarn House, High Newham Road, Stockton-On-Tees. The team is staffed by a Team Manager, Senior Social Workers, Social Workers, Social Care Officers and Administrative Officers.

The team responds to requests for assessments of need relating to adults over the age of 18 years who appear to have a need for care and support for a variety of reasons. The main aims of the team are to:
- Assess needs
- Prevent the escalation of needs/risks and promote well-being
- Minimise the impact of on-going needs upon the lives of those with long term conditions before referring onwards for support planning services where appropriate
- Delay the on-set of long term needs for formal support by promoting independence and working with people and their families to use community resources and informal networks effectively.
- Work in partnership with individuals, their families and Carers.
- Work in partnership with other services, teams and organisations across the public, private and voluntary sectors.

The successful candidate will be able manage a caseload of short term clients and will work within legislative frameworks to deliver an effective service to our customers. Organisational and problem-solving skills are therefore essential.

This post is open to both experienced and Newly Qualified Social Workers (NQSWs).
NQSWs will be supported to complete the Assessed and Supported Year in Employment.
