An exciting opportunity has arisen to join the Continuing Healthcare team as a Nurse Co-ordinator within Joint Commissioning in South Tyneside.

The South Tyneside Continuing Health Care team are an innovative and friendly team who deliver a service to the population of South Tyneside. This is a great opportunity to be involved in a team, who are working within the Local Authority, towards a more integrated approach in how we assess and meet an individual’s care needs.

We are looking for an experienced Registered Nurse to work within the CHC team to co-ordinate and lead Multi-Disciplinary Teams in applying the Continuing Healthcare framework in regards to eligibility for either funded Continuing Health Care or NHS Funded Care. Our nurses look at the whole process, first by determining eligibility for NHS funding through utilisation of the Decision Support Tool to Case Managing individuals ensuring identified care needs are being safely met.

The work is at times challenging but also rewarding. You will need excellent assessment skills and experience of working across a variety of care settings. The successful applicant will also need to demonstrate enhanced communication skills and have the ability to work both as part of a team and autonomously.
