An exciting opportunity has arisen in our Hospital Social Work Team, for a highly motivated social worker who is enthusiastic and committed to working in a centered/strengths based way with customers and carers. You will be expected To undertake direct work with adults, their families and informal networks and to work in accordance with the policies of the Council and Sunderland Safeguarding Board. We also offer an attractive incremental progression.

The duties of the post include: -
Undertaking assessments, care and support planning and reviews.
Multi agency work and close partnership work with Customers, Carers and other professionals.

Successful candidates will have-
Excellent communication and social care assessment skills, plus a solid understanding of the relevant statutory frameworks, and best practice.
Ability to work as an individual and within a busy social work team
Ability to meet targets and deadlines, including management of Data Quality.
The desire and motivation to work closely with health partners and other professionals.

A commitment to a flexible approach to their work to meet demands
Ability to work as an individual and within a busy multi-disciplinary team.
Ability to be flexible and self motivated
SW England registered.
