An exciting opportunity has arisen in South Tyneside Let’s Talk Service. We are seeking 2 experienced and suitably qualified people to fulfil the dedicated role of Adult Social Care Advisor (ASCA).

The Let’s Talk Service is the single point of contact into Adults and Integrated Care in South Tyneside and the ASCA is the first person a member of the public or a professional will communicate with.

Since the implementation of the Care Act 2014, Councils now have an extended role to play ensuring the provision of information and advice, shaping and providing support for people and their carers. Greater emphasis on individual outcomes has transformed the way that we deliver our service, enabling people to live at home for longer.

Here at South Tyneside ASCA’s have adopted an approach of providing Great Information and Simple Connections. The role of the ASCA is to listen effectively to what matters to the person fully maximising the benefits that great information can play in the delivery of meeting individual’s needs and outcomes. The role requires engagement and collaboration from across the system utilising the voluntary and community sector working together with a shared purpose.

Our growing focus is on prevention and early intervention working together in partnership with individuals, families, carers and our partner agencies in a more joined up way. To realise our vision we need to focus on making the most of individuals strengths and resources in the community looking at what people can do (rather than what they can’t) and focussing on what matters to them.

It is in this context that Great Information and Simple Connections is transforming the way our services operates. As an ASCA you will provide great information and advice to the individual utilising your knowledge of the social care landscape, you will provide this at the time they need it. You will be responsible for identifying when the person should be connected to another team (internal and external) based upon the complexity of need and risk. This means that the persons needs will be better coordinated, interventions will take place early, and the person will only need to tell their story once rather than multiple times.

The ASCA role is critical to ensuring that people are effectively connected to the right support for them, first time. Handling over 1000 calls per week, no 2 calls are ever the same and you will need to be an effective listener, able to act independently, be professionally curious and be committed to ensuring the person you are communicating with receives a personalised response.

You will respond to telephone, written and face-to-face communications. This may include carers, people with a learning disability, autism, physical or mental health issues and safeguarding concerns.

You will be central to the development, performance, quality assurance and continuous improvement of Adults and Integrated Care, ensuring services are responsive to the local and national landscape and contribute to the Council’s aims and objectives within an overall framework that safeguards adults, promoting health, independence and wellbeing.
