The Adults & Communities Directorate consists of three strands, Adult Social Care, Community Development, and Public Health. Our aims are to continue to provide essential and highly valued services and to build capacity to enable local communities to make lasting change, improve quality of life and prosperity. As a directorate we lead on developing a culture of co-operation and co-production, respecting and nurturing the skills and talents of local people and enabling them to fulfil their potential.

An exciting opportunity has arisen to join the Recovery and Independence Team. The service has changed its structure and is currently expanding and has introduced a career progression scheme.

The Team provides a short term (for up to six weeks) reablement service to people in their own homes which enables people to regain as much independence as possible in skills for daily living ( personal care, meal prep, medication, mobility, use of equipment, accessing the community etc).

The team also provide a rapid response service to vulnerable Adults in crisis.
This is a Short Term Care at Home service (up to seven days) to support hospital discharge and prevent admission to hospital or a care home.

The rota is a two week pattern of four days a week and three time periods in the Day:
Week one: Monday, Tuesday, Thursday (am only), Friday. Total 32.5hrs
Week Two: Wednesday, Thursday (pm only) Saturday, Sunday. Total 36hrs
Hours are worked in four separate time periods; 3hrs, 2hrs, 2hrs, 2.5 hrs

The successful candidate will: hold a minimum of a level 2 Diploma in Health and Social Care or equivalent; hold the HSC level 3 Medication Unit or have a commitment to achieve the unit; numeracy and literacy level one or equivalent; have experience of working in a care/health setting; have an understanding of the ethos of reablement, drive and have the use of a vehicle.
