As part of our commitment to improving quality and service redesign, we are looking for a qualified, experienced and dedicated social worker to join our Let’s Talk Team.

The team plays an important role in supporting a different way of working within South Tyneside, which is focused upon achieving positive outcomes for our residents. The Let’s Talk Team is the first point of contact for Adult Social Care; its focus is on timely short term interventions that will maximise the independence and well-being of an individual, drawing upon their own strengths and assets.

The team works consistently with our longer term local area teams using the 3 Conversational approaches to build relationships with people and helping them to identify individual outcomes which promote their wellbeing by making the best use of our resources, all which are at the heart of our Adult Social Care Strategy. Our aim is to work with all our partners and deliver Practice to be Proud of to support the people of South Tyneside to Live Better Lives.

The role will involve working closely with multi-disciplinary practitioners within the team and wider partners to ensure we deliver the best possible outcomes for the residents of South Tyneside.
