Several exciting opportunities have arisen within Adult Social Care for highly motivated, qualified, experienced social workers. The posts are fixed term for 6 months to support our hospital discharge pathways work. You will be enthusiastic and committed to working with customers and carers in a person centered way using a strengths based approach. You will be required to work in a variety of locations including hospital and community settings and be flexible in your ability to support a demand led approach to the deployment of staff resource. This includes the flexibility to work in a team which covers a period of 8am – 8pm, 7 days per week, if required.

The duties of the post include:-
:- Undertaking screening evaluation, assessments, support planning and reviews.
:- Multi agency work and close partnership work with Customers, Carers and other professionals.
Successful candidates will have-
:- Excellent communication and social care assessment skills, plus a solid understanding of the relevant statutory frameworks, (including current Government guidance on hospital discharge pathways).
:- Ability to evidence and demonstrate best practice.
:- Ability to work as an individual and within a busy social work team
:- Ability to meet targets and deadlines, including Data Quality.
:- The desire and motivation to work closely with health partners and other professionals.
:- Social Work qualification and be registered with SW England.
