North Tyneside council have the opportunity for you to join our Reablement Discharge Team in a support worker role. This is a team which incorporates support workers and community rehabilitation officers and work within the borough of North Tyneside visiting and supporting clients within their own home.

The reablement team consist of support workers, community rehabilitation officers, locality leads and scheduling officers. The team provide goal focused and outcome-based support within the community in the individuals own home. This support is to enable the individual to remain as independent as they can following an episode of illness/accident or health issue.

The service is about supporting people to do things for themselves rather than doing things for them. It is short time limited intervention of intensive activity to promote people’s independence. This involves working into individual’s homes supporting and promoting independence.

It involves; goal setting, a personalised approach with involvement from the individual and their family. It is a service that builds on what an individual can do already and support them to regain skills and build confidence.

The service works from 7.00am -11.00pm. These positions are permanent, 25 hours per week.
