The successful applicant will work as part of a team of Social Workers, Community Assessment Officers, and Occupational Therapists. The robust front door staff respond to initial contacts into Adult Social Care, such as requests for assessments of need, enquiries regarding safeguarding issues, advice and information about local services, and responding to urgent or crisis situations. The case worker element of the team, will enable the provision of short term social work with adults aged 18+ with a range of complex needs in the Darlington area, working proactively with service users, their families, carers and other relevant professionals. There are plans for piloting rotational work within the team functions in order to develop skills and enhance knowledge for all staff.

The dynamic, fast paced nature of the team is underpinned by a strong commitment to the principles underpinning The Care Act 2014 and other relevant legislation. We are looking for people who are committed to strength-based practice, focusing on enabling people as much as possible to build on their own networks and strengths as a means of resolving situations and meeting their needs. The successful applicants will also be able to, through gathering information from initial contacts, make appropriate and sound decisions as to what action needs to be taken, such as referring people for assessments under Care Act 2014, as to whether safeguarding issues are indicated, or signposting people to partner agencies and services. They will also be responsible for a range of assessments including: Care and Support Needs, Mental Capacity Act, Safeguarding Initial Enquiries, and Support Planning to promote the independence and wellbeing ensuring desired outcomes are achieved through person centred planning/strength-based assessment.
