An exciting opportunity has arisen for a Strategic Manager – Hospital Interface within the Adult Social Care service.

The successful candidate will be responsible for the strategic and operational management of the Hospital Interface Service, Care Home Team and Statutory Mental Health and Out of Hours functions in order to meet both the Directorate’s objectives and the strategic corporate objectives of the organisation.

You will be accountable for delivering assessment and care management as part of services to meet the needs of the population and improving the health & wellbeing for the citizens of Sunderland and the delivery of person-centred positive outcomes, with the aim of improving levels of independence for all.

The post holder should have strong experience of working with vulnerable adults in a front-line service delivery role and have the ability to create excellent working relationships across a multitude of partner agencies, in statutory, private and voluntary arenas. The successful candidate should have the ability to manage at a strategic level across all areas of the social care and health landscape locally and nationally, as well as a detailed knowledge of Out of Hours Social Work Provision & Operational Practice.

You will be responsible for a team and a budget, working with staff and customers to maximise outcomes and minimise financial demand.

This is a demanding role which requires high profile leadership to staff of all disciplines across the Service and ensuring the presence of a culture of continuous improvement within care services putting patients and their families at the heart of the service.

This post is exempt from the rehabilitation of Offenders Act 1974 and therefore will be subject to a DBS check from the Disclosure and Barring Service.
