Working alongside the other Assistant Team Managers, Advanced Practitioners and reporting to the Operations Manager, you will be an integral member of our multi-disciplinary Integrated Community Delivery Team, where excellent leadership, knowledge and skills are required to support our teams. This is both a challenging and rewarding post which requires a high degree of professionalism. Assigned to one of three area-based locality teams you will support a team of social care practitioners to undertake a range of strengths-based interventions to support our Let’s Talk Together approach which ensures our adults with care and support needs and their families and carers' achieve their desired outcomes.

We work with people and their families and carers', to ensure they remain a central part of what we do, to enable them to make choices and decisions about their lives. Focusing upon individual outcomes; putting the person in control and making the best use of our resources are at the heart of our Adult Social Care Strategy. Our aim is to work with all our partners and deliver Practice to be Proud of, to support the people of South Tyneside to Live Better Lives.

You must be registered as a Social Worker with Social Work England, to be considered for this post, have experience of managing a team and extensive knowledge of the Care Act, Mental Capacity Act and Human Rights Act.
