Sunderland Care and Support is a provider of high-quality care to the community of Sunderland, who are passionate about supporting adults with learning disabilities. We are seeking to recruit a Care coordinator (Level 1) into our Supported Living Division who can encourage a workplace culture based on respect and dignity for those in need of care and support.

Serlby & Serlby Bungalow
Serlby is a supported living service for (8) male adults with learning disabilities who are on the autism spectrum, who have extremely complex needs and may display challenging behaviours. Serlby Bungalow is attached to the main service. The bungalow is a bespoke service for (1) male customer who has learning and mental health disability who requires 2:1 support. Both services have Waking Night support, and Sleep-in provision. All the Customers living in both services require specialist support?to participate within their local community, develop a wide range of skills?with an aim of?maintaining?a tenancy within the city of Sunderland.?

As a Care Coordinator (Level 1), your focus will be to provide high value customer care across both services, maintaining and developing customer(s) quality of life by including them in meaningful activities, ensuring choice and support is given so they can access services to help them lead the life they want. This includes supporting individuals with a range of daily living skills such as preparing meals, completing household tasks, administering medication, finance management, accessing community activity, support with shopping, meeting friends & family, employment opportunities , and providing emotional support.

We are looking for Care Coordinators who are enthusiastic and motivated in their own personal development, you will have the ability to produce legible written reports and have good understanding of care plans. Your role will involve you in the absence of the team leader, being lead on shift to support your colleagues and act as an ambassador positive mentor for SCAS. You should have the ability to complete PMVA training. As an employee SCAS will support you on your journey via induction, probation, supervision and appraisal to ensure customers receive support which is designed to meet their own personal need.
