Community Planned Care Service
The Community Planned Care Team visit customers in their own homes and provide person centred care and support to people who may have a physical disability, mental health condition or learning disability. Support can include the provision of personal care, medication support, financial support and social support in the community. A full driving license is required and use of own vehicle.

The service operates a set rota featuring the following shifts:
7am – 3.30pm, 1.30pm – 10pm and alternate weekend working (no overnight shifts are required)
