Successful applicants will be supported via induction and training which relates specifically to the post for which they have applied. The post involves supporting adults who are recovering from illness and or trauma to recover and to gain independence through meaningful activity. Customers can be admitted from hospital or their own home and referrals are made via professionals within the City. The service is fast paced as customers stay only a short time before being supported to move to either their own homes or new accommodation depending on their needs. Our customers are often frail and frequently have comorbidities including varying degrees of physical and cognitive impairment. They often present as unwell and the team works closely with Multi-Disciplinary Team members including Social Workers, Therapists, Community Nurses, GP and Geriatrician. The role of support worker involves supporting the customer with all aspects of activities of daily living routines including following therapy plans for which training is also provided.
