The Pathways Leaving Care Team in Middlesbrough has an exciting opportunity to recruit new social workers to support young people in their transition in to adulthood.
Social workers within the Pathway teams have responsibility for working with young people who are looked after and care experienced young people.
The team provide an assessment of needs for young people who are preparing to leave care and the development of a pathway plan to meet these needs. They provide advice and guidance to young people who are eligible, relevant and former relevant under the Children Leaving Care Act 2000.
The team work closely with partners as corporate parents to support children to develop their life skills, achieve to their full potential and work towards their aspirations.
