Over the last 2 years we have been working to improve the support provided to adults over 18 who are experiencing multiple disadvantages due to homelessness, substance misuse, domestic abuse and sexual exploitation. We are now in a positon to pilot a new team which will be line managed by our Safeguarding Manager with close links to Middlesbrough’s new Integrated Services Model and our Making Every Adult Matter (MEAM) work.

The team will operate over a 12 month pilot to measure the effectiveness of a dedicated service who are able to work in a trauma informed way with increased flexibility to address system issues, We are hopeful that with proven success, funding will be extended for a further 12 months.

The Team will consist of a Principal Practitioner, Social Worker and Social Care Workers. The team will also have close links to the MEAM coordinator who is currently in post.

As the social care worker you will be expected to offer case work support to the Principal Practitioner, MEAM Coordinator and Social Worker. Your role will be based on an assertive outreach model, working to support and engage adults. This will involve offering practical advice and support to access services in addition to gathering relevant assessment information and developing a relationship of trust with the individual.
