The successful applicant must have a solution focused approach to problem solving and be committed to working alongside the manager to develop a highly effective and efficient service to meet the needs of both internal and external partners. This is an exciting opportunity for the successful post holders to join the service at the beginning of a period of change and service development.

You will work as part of a team to provide telephone triage, advice, information, assessments and rehabilitation, assistive technology solutions and rapid response to ensure that care planning and service reviews of customers their cares and equipment is of the highest standard whilst employing “Ways to Wellbeing Approach” to assessment. This be undertaken whilst adhering to regulatory standards relevant legislation, agreed eligibility criteria as well as managing and identifying risk, and adhering to Council policies and procedures.

You will be expected to work with colleagues across Housing, Social Care, Health, and Children’s Services. You must have a “can do” approach to managing challenges within the service or within assessment outcomes. You must ensure that both individual and team targets achieved. You must also have experience of successfully co-ordinating services for people with complex needs whilst working in partnership with both internal and external partners or agencies.

The Community Occupational Therapy Service is committed to supporting customers of our service, their families and cares with appropriate assessments and interventions whilst giving a tailored a response to support people to live independently, well and safeguarded.

Successful applicants will receive a planned induction, regular supervision and a variety of opportunities for continued professional development. They will also have access to an excellent range of in house and bespoke training in addition to joint training with health colleagues to help develop their career as an Occupational Therapist.
