An opportunity has arisen for two permanent, full time, Social Worker posts, to work in Community Health and Social Care Direct (CHSCD) for Newcastle City Council. The positions will be based in Community Health and Social Care Direct for at least the first year. CHSCD is the first access point for adult social care across Newcastle upon Tyne.
You should have a relevant social work qualification, such as Degree in Social Work, DipSW, CQSW, CSS or equivalent, as well as HCPC registration. We are happy to consider applications from progressed and un-progressed Social Workers as well as newly qualified / qualifying Social Workers.
The successful candidates will be responsible for a variety of tasks including responding to crisis/emergency situations, completing adult assessments, carers assessments and support planning, often in challenging situations. You will provide information, advice and support to prevent the need for long term support and promote independence. The role requires you to be flexible, adapt quickly and have the ability to make sound judgements, often in an emergency. The Community Health and Social Care Direct Team work in a multi-disciplinary environment with health colleagues and care workers. As such, the team has an ethos towards collaborative working.
