An exciting opportunity has arisen to join the Community Reablement Team for the reablement and support of people within Redcar and Cleveland area. Join us in providing essential, rapid response care within the comfort of a person's home for up to seven days. The rapid response service makes a real impact in people’s lives preventing hospital and care home admissions alongside supporting hospital discharges.

The team also supports people in their own homes for up to 6 weeks helping them to regain their ability and confidence to live independently. If you are an empathetic people-person who wants to support and make a real difference in the community, we want to hear from you.

Qualifications are not the most important thing; we can help you to gain the qualifications you will need to progress in your career with us.

ABOUT THE JOB The rota consists of  • Week one: Monday, Tuesday, Friday. • Week Two: Wednesday, Thursday, Saturday, Sunday.
