The team…

The Provider Service supports people with disabilities (learning disabilities, physical disabilities, or mental health conditions) to be influential in their local communities through citizenship, employment, enterprise, and volunteering opportunities. With a strong focus on the abilities of the individual and not their disabilities, we recognise the importance of supporting individuals to obtain their maximum potential and achieve the same life outcomes as those of people without disabilities.

 

In line with Gateshead’s continuing pledge to put people and their families at the heart of everything we do and our continued expansion of outcome focussed, strength-based practice, we are looking to recruit two Enablement Officers to work within Gateshead Council’s Enablement Service, PRIME (People Regaining Independence by means of Enablement). This is an exciting opportunity to join a well-established and experienced team that will provide short term support to individuals across all of adult social care who may be experiencing physical or cognitive difficulties.

About the role….

As an Enablement Officer you will work within the context of an enablement approach and operate within an integrated service to provide individualised, outcome led personalised support, based on structured individual assessments to minimise or eliminate a service user’s dependency on long term social care. This will involve conducting timely, regular and frequent reviews of people’s support plans through structured reassessments and put in place any necessary revisions to ensure that the plans remain, relevant, proportionate, appropriate and cost effective in delivering the required outcomes, in the context of changing circumstances.

In collaboration with line managers, you will be required to achieve full compliance in governance requirements within the service.  To meet the essential criteria for this post you will need an NVQ level 3 in Care or willingness to work towards it.
