Pioneering Care Partnership is proud to be part of a multi-agency partnership, delivering support for people who present with poor mental health.
Community Health Transformation in County Durham (CMHT) is a multi-agency approach to achieving system change in the delivery of mental health services.
The CMHT programme seeks to revolutionise how services are delivered, and key to this is empowering people to have greater choice and control over their care, and to live well in their communities. Pioneering Care Partnership is working with other partner organisations to support this CMHT process and establishing both Lived Experience and Peer Support Worker structures.
We are looking to appoint a forward-thinking, enthusiastic and experienced
individual to lead and develop this agenda. The successful candidate will ideally have a Level 4 qualification in health/social care or community development, have experience of leading people within multi sector teams in the community and ultimately have a passion for supporting people into health and wellbeing services.
The successful candidate will manage a team of community-based Peer
Support Workers, employed by the voluntary sector, to support this process.
