About the Team….

Our assessment teams use a conversational approach, focusing on an individual’s strengths and the community around them, supporting Gateshead residents to lead thriving lives.  Gateshead’s Achieving Change Together (ACT) Team supports adults with complex and long-term needs through in-depth reviews, exploring creative approaches which promote people’s wellbeing and control.  We harness community assets, assistive technology and enablement to ensure that people are supported in the least restrictive way possible.

 

The Role….

ACT Enablement Officers play a key role in providing bespoke, time-limited support to enable people to achieve their outcomes and aspirations.  You will work alongside people and their caregivers as well as other professionals and community partners to explore their goals and develop individualised, outcome-focused enablement plans which build on skills, confidence and independence.

 

You will undertake timely reviews of people’s enablement support plans and measure progress towards their outcomes through structured conversations - making any necessary revisions to ensure plans remain relevant, proportionate and cost effective.  You will also assist in supervising our enablement support workers and managing staff rotas, as well as providing direct enablement support when required. 

 

With experience of working collaboratively to support adults with a wide range of needs, you will be committed to working in a person centred and strengths-based way.  You must have good IT skills together with excellent communication and written skills.

You’ll be paid a competitive salary , Grade H starting on £35,235 rising to £37,938 per annum
