The team…

The Provider Service supports people with disabilities (learning disabilities, physical disabilities, or mental health conditions) to be influential in their local communities through citizenship, employment, enterprise, and volunteering opportunities. With a strong focus on the abilities of the individual and not their disabilities, we recognise the importance of supporting individuals to obtain their maximum potential and achieve the same life outcomes as those of people without disabilities.

 

Provider services are also key to supporting older people to remain in their own homes through facilitating seamless hospital discharges, preventing unnecessary admissions to hospital and residential care, by working with individuals to regain or maintain their independence with enablement and reablement approaches.

About the role….

Sister Winifred Laver Promoting Independence Centre provides 24/7  short-term care services for adults,  leaving hospital or admitted from the local community, aimed at helping individuals  to regain the skills and confidence they need to go back to living their lives independently in their own homes. The care offered at the centre enables people to return home much quicker and can avoid people having to go into long term residential care.

 

You will be required to promote the independence of the service user whilst respecting their rights to privacy, dignity, and choice, whilst being aware at all times of their emotional and social wellbeing.

 

You will be required to provide personal care and support to service users, maintain and update care plans as goals are set and met under the guidance and supervision of managers.

To liaise and communicate effectively with other colleagues, service users, carers, and other professionals to share knowledge, seek advice and co-ordinate service delivery.

You’ll be paid a competitive salary £24,702 pro rata (31.5hrs per week) plus additional allowances where relevant.

You’ll get a generous annual leave package starting at 26 days annual leave (+ 2 statutory days) for new employees, rising to 30 days (+ 2 statutory days) after 5 years continuous local government service for full time workers. (pro rata for part time hours)

Hours of work will be 9.30pm to 7.30am on a shift basis.
