Within the Children’s Disability Team, we want employees who are passionate about disabled children and young people getting the right help at the right time, listening to their voice and working in partnership with families and wider networks. 

This is an exciting time to join us as we develop this new role of Reviewing Support Worker to add to the existing team which comprises of a Team Manager, Advanced Practitioner and 8 social workers. 

Working closely with our colleagues in Education and Health, the Children’s Disability Team aims to safeguard and promote the welfare and development of disabled children, whilst enabling them to be cared for by their families at home. We provide families with support to manage the care of children with severe and permanent disabilities.

As a Reviewing Support Worker, you will work directly with children and their families to assess and review their needs on a six-monthly basis, identifying packages of support to assist them. You will be conducting home visits, reviewing care packages across a multidisciplinary network and presenting families' needs to request packages of support.

The post is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 and an Enhanced DBS disclosure, including Barred List Check, will be required.
