Vacancy No: CAS/14/82A

Applications are invited for the above post based within the Integrated Short-term Intervention Service (ISIS) Team covering the South Durham area.

The post holder will have a key role in ensuring appropriate and timely response to assessment for clients in crisis, minimising unnecessary hospital admissions, supporting timely discharge from hospital.

The team enjoys excellent working relationships with colleagues in health settings, has close working relationships with the other Integrated teams and other statutory and non statutory organisations.

Applicants will need to demonstrate:
- excellent time management and organisational skills;
- their experience and enthusiasm in working flexibly with in a multi-disciplinary team;
- ability to undertake holistic assessment of needs, commission services, implement care plans, monitor and review.

It is anticipated that interviews will be held on 17/10/14.

Required qualifications: Professional Qualification: Degree in SW, Dip SW, CSS or CQSW or equivalent . Plus HCPC registration or be applying for HCPC registration.
