The ‘Horden Together’ project aims to deliver new ways of working that supports the needs of our most vulnerable people and delivers longer term solutions for the local community.

We are looking to recruit a dynamic Team Leader (MEAM) who shares the Safe Durham Partnership’s vision and can demonstrate our organisation’s core values and behaviour. We are looking for a highly proactive and enthusiastic individual who can help connect people with a range of services and improve the wellbeing of individuals, reduce reoffending, and influence future service commissioning and delivery.

The post holder will have a key role in the development and delivery of place based services around ‘Making Every Adult Matter’ to strengthen our existing partnership arrangements and facilitate step change in future service provision.

The successful candidate will have previous extensive experience of case management work and the development of personal action plans, intensive interventions and support to addressing complex needs and dealing with challenging individuals as well as having experience of providing supervision to a team of front line operational staff.
