VACANCY NO: AHS/22/852A

We are looking to recruit a permanent 37hrs Social Worker within the Durham and Chester Le Street Locality Team.

The Team enjoys excellent working relationships with colleagues in Health and other statutory and non-statutory organisations.

The social worker is responsible for completing assessments of need care planning and reviewing and working within multi-disciplinary settings for Adults with a physical disability and older people.

It is expected that the successful candidate will have experience of working within Adult social care together with an up to date knowledge of multi-agency working and the associated adult care agenda.

Required qualifications: Professional qualification: Degree in SW, Dip SW, CSS or CQSW or equivalent, plus Social Work England registration ( or be applying for registration).

Candidates who do not evidence that they meet the essential qualifications and experience listed on the person specification will not be short-listed

Durham County Council currently operates a hybrid working policy where a model of 2 days in the workplace and 3 days at home/remote will apply.
