The Community Reablement Service assists people living in the community to regain skills and confidence that may have been lost due to an illness or physical disability following a period in hospital or residential care. The Reablement Service will assist for a time limited period and set goals to promote independence and wellbeing.
Purpose of Role
• To assist the management team with initial assessment of a customer to determine their requirements and to ensure the customer receives the right type and level of care and support to meet their needs
• To lead on the customer care and support planning process
• To work with the customer to set goals and provide person cantered care and support
• Assist the manager of the service in terms of team working, ensuring tasks are carried out accurately and efficiently, interventions are appropriately conducted, staffing resources deployed to meet customer need and with the development of colleagues.

You will be required to work independently in the Community and have the use of your own vehicle with business use insurance.
The job role includes working shift patterns including weekends and bank holidays.
