Are you a tenacious and patient individual who can support people through complex challenges?
We are looking for five dynamic and tenacious Changing Futures Keyworkers, to be based in Middlesbrough within the housing team, working with people in supported accommodation.
As a Changing Futures Keyworker, you will be passionate about making a difference to people’s lives, providing a person-centred approach to support positive lifestyle changes for the people you will work with.
These posts are 3-year fixed term.
In this role, you will:
- Provide targeted and intensive support to individuals experiencing multiple disadvantage and communities through persistent engagement
- Bring together communities and key partners to share resources and build on existing strengths and assets to deliver improved outcomes
- Coordinate partner agencies and specialist services to effect and sustain positive change for individuals by acting as the interface between them and partner agencies
