Applications are invited who have appropriate experience and who can provide direct care and support to adults with complex health needs and associated disabilities within the Orchard Day Service. Additionally applicants must also have experience of working in partnership with a senior team to deliver and review individual's personal programs.

The successful person will need to demonstrate an ability to work within complex and rapidly changing environments, as well has having the ability to identify any potential threats/risks, taking active steps to rectify and overcome them.

Applicants must:

To have an appropriate NVQ 2 or equivalent in Care, with the HSC375 Medication Unit as a desirable requirement, but not essential.
To have achieved a good standard of education to GCSE level or equivalent
Have substantial practitioner experience in the field of Social Care working with people with Complex Health Needs
Knowledge and understanding of a range of disability issues
Have a some understanding of the principles within Person Centred services
To demonstrate an ability to exceed professional standards and act as a role model for new qualified staff

You will also be required to work a flexible working rota, which may include evenings or weekends. A full UK Driving License is a desirable requirement of this post.
